This is a further interesting illustration of the nonreversibility of resistance in the absence of the selecting effect of the drug. The drug-resistant phenotypes do not seem to be at any disadvantage in competition with their drug-susceptible relatives. Sulfonamide-resistant strains of S. pyogenes were shown to have very high resistance, some of them with MIC values of 1 mg/mL. The mechanism of resistance in S. pyogenes turned out to be related to the sulfonamide resistance of N. meningitidis, in that it is based on the horizontal transfer of genetic material.

Earlier, a large part of the antibiotics consumption was used as feed additives in husbandry and to some extent also in plant agriculture. The use in animals was based on the empirical but not completely understood observations that meat animals gained weight faster when given antibiotics in their fodder. It soon became clear, however, that this practice led to the spread of antibiotic resistance through the food chain into the general population. An example of this was the use of avoparcin, a glucopeptide, an analog to vancomycin (see Chapter 5).

In Scandinavia, the distribution of sulfonamides as a single drug for systemic use is presently nil. Aside from preparations for external use, as in ointments, the minimal distribution of sulfonamides that still occurs is in combination with trimethoprim. There are three primary reasons for the almost nonexistent use of sulfonamides. First, other and in many cases more efficient antibacterial drugs became available through the decades following the introduction of sulfonamides in 1935. Second, resistance emerged rapidly in many pathogens.
